The focus of this work was to plan a network of facilities that will offer accountable patient-centered care system that will improve population health outcomes in a more cost-effective way.
1080 (formerly CITE360studio) engaged the client and stakeholder groups in a series of workshops and discussions to provide a Lean-informed planning and analysis process. Through strategic planning and consultation, it was determined that a shift in attitude toward the delivery of healthcare to the community was desired. We proposed the implementation of community ‘hubs’ to gather the Saskatchewan Health Authority’s (SHA) desired services, with each hub positioned to address ‘Hot Spot’ zones that would offer the greatest potential impact on reducing costly acute care.
Following the completion of this phase of work, our team has maintained a working relationship with the SHA project team, and continues to support this initiative as it enters its next phases of planning and design.
